Note: Due to U.S. Department of Homeland Security requirements, our travel agent must submit to airlines copies of all NP:J participants’ passport photo pages 60 days prior to departure. Please be sure to give a copy of your passport photo page to your
teacher by March 2, 2009 so that NP:J will receive your group’s passport copies in a timely manner. Any copies received late will
require special administrative attention thereby incurring a $50 late fee (please refer to the Important Details document).
If you are applying for a new or renewal passport, submit your passport application IMMEDIATELY upon acceptance to NP:J. It may take 3 months to
get a passport. Students who are not U.S. or Canadian citizens need to contact the nearest Japanese Consulate to find out if a tourist visa is required.

Payment Information
The NP:J fee is US$3,550 plus an airline surcharge of US$245 ( fees will vary based on departure airport and airline policy).
A down payment of US$250 per student participant is required with this application. For refund/cancellation policies, please refer to the Important Details document.
After submitting the down payment, the balance of the program fee is payable in four equal installments. If the balance is paid in full on the first installment date ( January 15, 2009 ), you may deduct US$50 from the total NP:J program fee. Please write student’s full name on every check,
made payable to The Laurasian Institution.
I will turn in my down payment of $250 and a signed Letter of Understanding to my teacher by the application deadline, November 24, 2008.

Scholarship Information
The NP:J Jackson Bailey Scholarship is a one-time award to qualified students, based upon a combination of merit and need. Students who
apply to NP:J and meet the program and scholarship criteria as outlined in the NP:J program brochure are eligible to apply. All scholarship
recipients will be required to submit to NP:J a post-study tour project of his or her choosing.

SCHOLARSHIP DATA:
1. NECESSARY ATTACHMENTS: In order to be considered for an NP:J Jackson Bailey Scholarship, please physically mail the following documents to this application:The Laurasian Institution, Attn: NP:J, 12345 Lake City Way NE #151, Seattle, WA 98125
- A written recommendation from someone other than a family member who has acted in a supervisory role.
- A photocopy of the first page of the most recent 1040 tax form (or equivalent) filed by each parent in the household, which shows both TOTAL INCOME and NUMBER OF DEPENDENTS claimed for tax purposes.
- An official copy of your school transcript.
- Essays described on the bottom of this application.
2. ESSAYS: Please submit a typed hardcopy of the below essay questions:
- Why should a scholarship be awarded to you? (In other words, why are you a worthy and unique scholarship recipient?)
- What are your plans for earning and saving money for this experience?
- If selected for a scholarship, when you return from your NP:J trip, what type of project or report do you plan to submit about your NP:J experience? ( You may do a group project with other classmates if you prefer. Scholarship recipients must submit this report or project to TLI by November 30, 2009.)
